Purpose of this assignment:
The aim of this assignment is to enable the learner to demonstrate understanding of the processes, roles and responsibilities of health and social care professionals and organisations who may work collaboratively to provide for the care needs of individuals. Knowledge regarding the promotion and evaluation of partnership working within the health and social care sector and how partnership working can influence power sharing, choice and independence will also be assessed. Scenario:
Susan is eight years old. She lives with her mother Vicky, father Bryan and six years old twin brothers, John and James. The family is struggling to make ends meet and the council property has not been well maintained. Mould within the walls of the house, broken shower and kitchen cupboards along with improper insulation characterise the dwelling. Susan has been diagnosed with Cerebral Palsy and her father, Bryan has started to drink. Her mother Vicky has two jobs and the father and mother who does not drive get some help with transportation from Jenny (Vicky’s sister). Other relatives help with taking care of Susan.
The twin’s teacher has been complaining about the challenging behaviour being exhibited by them. They have been diagnosed with ADHD and Vicky has been threatened with their removal from their current ‘mainstream’ school. Jenny, although willing to help has been complaining about problems with parking as they live on a busy street and most of Susan’s appointments are in the centre of the city. Vicky has recently been having problems at work when asked for time off to care for Susan. Vicky has just fallen out with Jenny because Vicky has bruises all over her body which Jenny suspects (from past experiences), has been caused by her husband Bryan. Vicky refuses to comment.
As a care worker on a training programme your are required to use the case study provided to write an essay which will identify and explain the roles of the impacting health and social care professionals along with some obligatory organisations and multi-disciplinary teams who would be involved in supporting the service users, their families and their carers.
